METHOD STATEMENT FOR:


Installation of CCTV system

SCOPE:

To brief supervisors/workers on the safe work practices of planned works for upgrading the Security system of Mattel site.

DUTIES & RESPONSIBILITIES

1. Supervisors - Ensure method statement has been briefed to workers


2. Workers - Attend toolbox meetings, briefing for safe work procedure

PPE PROVIDED
TYPE OF TRAINING REQUIRED
1. Safety Helmet 4. Hand Gloves
1. All workers have followed directions by GC or FM.
2. Safety Shoes 5. Health mask
2. All workers to be properly trained
3. Safety Belt

WORK PROCESS/ ACTIVITY/ PROCEDURE:


Normal:

1. The working area to barricade to prevent other unauthorized from accident.

2. All the workers must wear necessary PPE. (Safety helmet with chin strap, safety shoes, safety belt, hand gloves)

3. Check ladder to be used is not defective. Ensure the stability of the ladder when placed against
structure or else one worker is required to hold the ladder to ensure stability. Worker is not to stand on the
last two steps of the ladder.

4. To be extra careful or use 2 workers when carrying or moving long materials.

5. To ensure that A-Frame ladder is safe to use, and 1 person shall hold the ladder

6. To ensure passing of tools shall be hand to hand and shall not throw.

7. To ensure that no unused tools left on the ladder.

8. Make sure workers are wearing proper PPE and safety belts are anchor at all times when working high level

9. Axindo will do cabling patch cord for CCTV.

10. Axindo will do the set-up and configuration of the devices offsite and request all the IP address from client to be programmed and
confirm the location of the main server.

11. Axindo will install the new camera devices and installation of hanging bracket camera.

12. Axindo will confirm the new cameras CCTV devices are working locally to the Mattel site
