This is to CERTIFY that quoted hereunder is the official Blotter /Entry


specifically reflected in the Police Blotter of this Station entered on May 20, 2022 the
following Entry exists to wit;

Report of Lost Wallet


(Non-crime)

That on or about 10:00 PM of May 20, 2022, one(1) Michelle Grace Santiago
Kalaw, 39 y/o, single and a resident of Brgy Uno Santa Maria, Laguna personally
appeared before this office and reported that her Philippine Passport wit No.
EC0257425 issued on February 11, 2014 with expiry date of February 10, 2019 and
Philhealth ID no.02-02-6646386-4 was lost/missing without her knowledge.
According to the reportee sometimes on May 15, 2022 she left at her residence and
went to Brgy Matalatala, Mabitac, Laguna on board a tricycle and when she arrived
home at the above-mentioned place she noticed that her passport was no longer
inside her bag and believe to be fallen somewhere while on her way home.

Despite of diligent search and efforts to locate the said Passport it could no
longer find and believe that it is now lost beyond recovery .

Issued this 20th day of May 2022 in Mabitac, Laguna, Philippines for whatever
legal purposes it may serve.
