effectiveness of the Supervised Industry Training (SIT) or On the Job
Training (OJT) you had with the Industry Partners of CENTECH-
Center for Technical Excellence, Inc. Please check (/ ) the appropriate

of this evaluation shall serve as a basis for improving the design and
