SUBJECT : Investigation Report re- Lost of Security and Company ID’s

1. References: Affidavit of Loss
2. This pertains to the above-mentioned incident that was discovered at
about 7:30 PM of December 3, 2024 at #160 Blk 12 Lt 3 Ph 1, Bagong Nayon 2,
Barangay San Isidro, Antipolo City Rizal, wherein the reporting person was identified
as EDGAR CANJA y Varona, 51 years old, born on October 18, 1973 in
Valenzuela, male, married, MCGI, security guard, and resident of at #160 Blk 12 Lt 3
Ph 1, Bagong Nayon 2, Barangay San Isidro, Antipolo City Rizal, wherein the
missing ID’s were to wit;
a. One (1) Security Identification Card issued by the Philippine National
Police; and
b. Company Identification Card issued by Sinamar.
3. That on December 3, 2024, when the reportee got home he noticed that
his Security Uniform together with his ID’s are missing. Thereafter, he figure out that
when on his way home, his back pack was accidentally opened and the said items
was fell out of his back pack. Further, the reportee exerted diligent efforts to locate
the said items but found futile.
