ASSESSMENT SCRIPT

❖​ GREETINGS

ORIOS: Good day, candidates. Welcome to CT-303, your accredited


assessment centre for the Extension Wire Making. My name is Christine Angelica M.
Orios, and I am with my co-assessor, Francis Diane Abayata. We are an accredited
competency assessor for Extension Wire Making NC II, and our IDs and accreditation
numbers are here. Also, our appointment letter is coming from the AC manager of this
institution. We are an accredited assessor for this assessment.

❖​ ACKNOWLEDGEMENT

ORIOS: Let me acknowledge first the presence of Mr. Rolland Gou, our AC Manager
and Mr. Rushdee Cubian, our TESDA REPRESENTATIVE from the TESDA
PROVINCIAL OFFICE. You don't have to worry about their presence because
are just here to observe if the conduct of the assessment is by the
competency assessment methodologies and procedures.

Check the Attendance of the Candidate:

l Collect the admission slip.

l Instruct the candidate to sign the attendance sheet.

❖​ ATTENDANCE

ABAYATA: I will now check your attendance. As I call your name, please come forward
and present your ADMISSION SLIP to me, and please sign the attendance sheet. Mr.
Nacito please come forward, Mr. Saquibal, Mr. Kiay.​

l Check the completeness of the rated self-assessment guide.


ABAYATA: I have the accomplished self-assessment guide with me. This indicates that
you know and can perform all the units of competency enumerated in this qualification.

Provide an overview of the Qualification to be assessed (National Assessment


and Certification)

l Explain the context, purpose, and the process of the assessment

ABAYATA: Today, we will administer the EXTENSION WIRE MAKING NC II. This
assessment has three assessment types: written examination, demonstration, and oral
questioning. These assessment methods will help us collect evidence to assess your
underpinning knowledge, skills, and overall competency.

ABAYATA: The Qualification or the Unit of Competencies to be assessed are namely:

1.​ Measuring and cutting electrical

2. Stripping, connecting, and insulating wires

3. Assembling plugs, sockets, and safety devices

4. Testing extension wire

l Outline the assessment procedure to be undertaken.

ABAYATA: The written assessment will be conducted here, the demonstration


assessment will take place on your right side, you will be given 10 minutes to perform
the extension wire making. After the demonstration, you will return here for the oral
questioning. All assessments will be completed within 30 minutes.

NOTE: proceed na dayon sa one-on-one interview, which is kailangan lagyo ang first 2
person na e interview [lagyo og area si first assessee, 2nd assessee, then last na e
interview si 3rd assessee so pwede ra siya sa gawas sa mag stay]

NOTE: Proceed sa interview questions pili lang pila kabuok e question. Pwede rsd mag
proceed sa MCQ after one-on-one interview para maka less og oras tapos to be
followlong lng sd ang last assessee na e interview.
l Assess the needs of the candidates and determine any allowable
adjustments in the assessment procedure.

❖​ PERSONAL NEEDS

ORIOS: The comfort room is beside the lobby for your personal needs. You can also eat
in our canteen on the 1st floor. However, during the assessment you are not allowed to
leave the premises of the assessment center. But before we forget, does anybody here
who has Physical difficulty or health issues? Please tell me so that I can assist you.

l Seek feedback regarding the candidates understanding of the


Qualification/units of competency to be assessed, evidence requirements,
and assessment process.

❖​ ALLOWABLE OR REASONABLE ADJUSTMENT WHEN APPLICABLE

ORIOS: If you have difficulty in understanding the given instruction, please don't
hesitate to call me or my co-assessor’s attention because we are more than willing to
explain and translate it for you.

l Specify the DOs and DON’Ts inside the assessment venue.

❖​ OSH
ORIOS: If you have concerns, clarifications, and questions, please raise your
hand so that I can answer them because once the assessment has started, it's no
longer allowed. In addition to that, cell phones are not permitted in the assessment
area. Please turn off your phone and put it in your bag.

ORIOS: Please observe safety practices as we go along with our assessment; Also,
when accidents happen or occur, such as burns, cuts, etc, please call my attention
immediately so we can give you a first aid kit. When the need arises, remember that our
first aid kit is located on the right side of the assessment area.

l Instruct the candidates to accomplish the competency assessment


agreement.

❖​ ISSUANCE OF N.C.

ORIOS: If you have found it to be COMPETENT, for the issuance of this


NATIONAL CERTIFICATE: You can claim your national certificate within three working
days after the assessment and process it in one day. Just bring 50 pesos for the
processing fee and one passport-size picture with a white background.

❖​ RIGHTS AND APPEAL SYSTEM AND RE-ASSESSMENT PROCEDURE

ABAYATA: If you are found to be COMPETENT for the issuance of this national
certificate or COC, the AC manager will inform you where you can get your NC or COC
five working days after the assessment. Just bring 50 pesos for the processing fee and
one passport-size picture with a white background.

ABAYATA: However, if you are not competent in some areas of the qualification, you
must be reassessed to the competencies you have found to be NOT YET
COMPETENT.

ABAYATA: If you have failed 2 consecutive assessments, you will be advised to get a
refresher or you can apply for reassessment, okay? In case you are not satisfied with
the result you can file for an appeal; you can send your letter to the TESDA provincial
office addressed to our TESDA DIRECTOR. Our (TESDA) Davao del Sur Provincial
Office is located at Aquino Subd, 617 Int 2 Rimas St, J.P. Laurel Ave, Davao City. Did
you understand?

During Assessment

❖​ PROVIDE EACH CANDIDATE A COPY OF “SPECIFIC INSTRUCTION TO THE


CANDIDATE”

ORIOS: Now, do you have any questions? Since you don't have any questions. I
will now give you your specific instructions, so please read the instructions carefully,
Please ask us if ever you don't understand something so we may be able to translate
or rephrase the instructions for your convenience.

❖​ ASSIGN NUMBERS TO CANDIDATE

ORIOS: Each candidate will be provided with numbers. Mr. _________, you will be no.
1, and Mr. ___________, you will be candidate no. 2. And Mr. _________ will be the
candidate no. 3. Ok, so candidate no. (1/2/3) Please answer these self-assessment
questions to determine if you possess the basic knowledge on
The expertise you are about to take.
ABAYATA: Let’s start with a written test. (Distribute the written test.) This will test your basic
knowledge of the tools, materials, and extension wire-making processes. Please read the
questions carefully and choose the best answer. You will have 5 minutes to complete the test;
you may now start.

ABAYATA: The 5 minutes are over; I will now collect your test questionnaires. Thank you.

ABAYATA: For the second part of our assessment, we will now proceed to the demonstration.
Candidates 1,2 and 3 please proceed to the assessment area.

ORIOS: In the demonstration, you will have 10 minutes to complete the task. You will be tasked
with extension wire making by following the proper steps, including measuring and cutting
electrical, stripping, connecting, insulating wires, assembling plugs, sockets, safety
devices, and testing extension wire. During the demonstration, I will assess your
performance using the observation checklist. Do you have any questions about the instructions?
None? That’s good. Remember, once the assessment has started, you must remain in the
assessment area for the entire duration of the activity. You may now begin.

l Provide the candidates with complete materials, tools, and equipment required to
complete the task.

l Administer the assessment in accordance with competency based assessment


methodology.

l Give specific instructions to the candidate clearly and slowly.

a. Observance of specified time limits provided in the assessment package.

b. Translation of written and verbal instruction to local dialect as needed.

c. Allow candidates to raise a question before the start of the assessment.

d. Reminding the candidate to remain in the assessment area during the entire
duration of the activity.

Record details of evidence collected and make judgement about the candidate’s
competence based on collected evidence.

ORIOS: The 10 minutes are over. Let’s now proceed to the oral questioning. Please take your
seats for the instruction. In the oral questioning, Candidate No. 1, Mr. Nacito, you will be
assessed by Ms. Abayata. Please take this seat (provide a seat, make it far from the other
candidate as far as possible). Candidate No. 2, Mr. Kiay please take this seat; you will be
assessed by me. Candidate No. 3, Mr. Saquibal, please proceed to the waiting area. We will call
your number when it’s your turn to be assessed.

ORIOS: The feedback will be one-on-one after performing the extension wire-making,
and it will be held inside the room for at least more or less than 5 minutes. After
performing the extension wire making, the assessor will evaluate based on their
performance output, and they need to get at least 8 out of 10 in the cognitive exam to
be marked as SATISFACTORY while 10 out of 10 for aiming to be marked as
COMPETENT

Post-Assessment

l Provide feedback on the outcome of the assessment process.

a. Clear and constructive feedback on the assessment decision.

b. Information on ways of overcoming any identified gaps in competency


revealed by assessment.

(Possible Feedback)

Competent Feedback: I am satisfied with your performance. You have demonstrated


the necessary skills and knowledge effectively. Keep up the good work, and
continue refining your expertise.

Not Competent Feedback: I appreciate your effort in this assessment. While


unsatisfied with your recognition, I recognize your potential to improve. Some
areas need enhancement to meet the required standards. I encourage you to focus
on these areas and seek guidance to develop your skills. Keep going, and learning
is a continuous process!

l Prepared the following reports

a. Record the assessment outcome using the prescribed rating sheet.

b. Check the completeness and accuracy of signed reports or rating sheets.

c. Prepare the recommendations for issuing the national certificate or certificate


of competency.

Furthermore, after declaring their scores, each assessee can claim their NC II at the
TESDA provincial office in 3 working days after today. Just bring 50 pesos and a
passport-size picture. Please sign in the CARS, and the assessor will sign, too.
On the other hand, if an assessee is marked as NOT SATISFACTORY for the
demonstration and will result as NOT YET COMPETENT, they will still receive their
COC and are welcome to reassess at their convenience at any assessment center of
their choice. Hence, if unsatisfied, they can write a letter of appeal within 24 hours
addressed to the TESDA director as an alternative. If there are no other concerns or
clarifications and they agree to the given assessment, allow them to affix their signature.

ABAYATA: We have now completed the assessment. Congratulations to those who


passed. Those who did not meet the required standards are encouraged to undergo
re-assessment once they feel adequately prepared.

ABAYATA: Again, to those who passed, the AC manager will inform you where you can
claim your NC or COC five working days after the assessment. Please bring 50 pesos
for the processing fee and one passport-size picture with a white background.

ORIOS: Are there any questions or concerns before we officially conclude the
assessment? If there is none, that concludes our assessment. Congratulations
